your contract number
what happened (describe the event in couple sentences)
when did it happen (date and time)
what is estimated value of the damage
what was damaged
if you have photos, please attach them

Our colleagues from claim dept. will register the claim and you will receive confirmation, claim number and list of other documents if required.
